Role 
The role of the QIDP is to coordinate and ensure the implementation of the active treatment program for each individual client within the organization's three (3) 6-bed Intermediate Care Facilities (ICFs). 

The role of the QIDP includes the administrative functions necessary to manage the personnel, funding, and ensure the implementation of the program design. 

Responsibilities

  • Ensure the performance of an annual comprehensive functional assessment and the ensuing development of an individual service plan for each client. 
  • Ensure and monitor the implementation of each clients’ Individual Support Plan (ISP). 
  • Provide training to the staff responsible for the implementation of each ISP.  
  • Ensure that staff are knowledgeable in the interventions necessary to implement the ISPs. 
  • Perform evaluation, revision, and monitoring of client ISP and active treatment programs.  
  • Ensure that all client needs are assessed and addressed through active treatment services. 
  • Coordinate personnel provision and training with Assistant Director to meet active treatment program needs. 
  • Support the implementation of the Focus system of active treatment through its implementation, role model behavior, and staff training. 
  • Coordinate the consultant evaluations of clients and the functioning of the Interdisciplinary Team. 
  • Coordinate and ensure staff training by the consultants. 
  • Investigate and provide a plan of correction for all incidents. 
  • Monitor the implementation of the facility policies and procedures and ensure implementation of licensing standards. 
  • Ensure funding and licensing requirements are always met. 
  • Monitor functioning of other departments such as dietary, ancillary, maintenance, health services, etc. to implement the program design and meet requirements of the program design and facility standards. 
  • Coordinate all interdepartmental, interdisciplinary, and Interagency communication and functioning.
  • Safeguard the spending of resident monies, personal & incidental funds, and allotted funding. 

Tasks:

The QIDP shall perform such tasks as needed to fulfill the functions, role, and responsibilities as required in the applicable regulations and as set forth in this job description. 

What We Need From You

  • 6 months experience working in direct care of persons with developmental disabilities, or a related experience such as elder/dependent adult care, mental health or social work. 
  • A BS/BA degree or higher in Human Services, Social Work, Mental Health or Nursing.
  • Possess a current and valid driver license and willingness to drive, both personal and company vehicles, to fulfill duties in office and at the ICF homes.
  • Ability to clear a criminal background check with the Department of Public Health.
  • Complete medication administration training and certification (conducted in-house)
  • Ability to respond to calls/texts 24/7 on a progressive basis.
  • Ability to pass a physical exam (push, pull, lift up to 50 lbs, stand for extended periods).
  • Physical condition sufficient to engage in frequent to continuous standing, walking, lifting, carrying objects, assisting with transfers, pushing pulling, bending, kneeling, squatting, reaching, running, and managing aggressive behaviors. These activities involve normal housework and assisting individuals who may engage in physical aggression.
  • Ability to sit or stand for extended periods
  • Ability to respond to any and all situations that occur within the facility to ensure the safety of all clients and staff.
  • Ability to speak, read and write in English
  • Attainment of full covid-19 and flu vaccination.

Evaluates:
Provide input to Assistant Director of ICF and Director of ICF in all Direct Care Staff reviews. Evaluates the Registered Nurse Consultant and the Assistant Director of ICF.

Hours:
Salary position, as necessary to fulfill job responsibilities and required by the Executive Director.
